A Two-Tier Model Healthcare Solution


Live Long and Prosper, America (LLAPA) is a 21st Century healthcare solution that connects a fragmented system, automates many administrative tasks, and incentivizes healthy outcomes. Plans such as Medicare For All would upend the insurance industry, wreak havoc on the stock market, create doctor shortages, and cause massive layoffs for an unproven system. LLAPA is a more measured approach. Using the Two-Tier model popular in such countries as Israel and Ireland, LLAPA addresses the core problems in our healthcare system while lowering the cost of healthcare by 25% or more. 

A Two-Tier Health System is a public/private partnership that ensures a healthy and productive population while maintaining the free market for the majority of healthcare services. In Tier 1, the federal government provides universal access to basic care including, for example, prevention and education services, vaccines, generic prescriptions, prenatal care, mental health and addiction treatment, and catastrophic care.

Providing free basic care eliminates surprise billing and medical bankruptcies, ensures access to life-saving medications, and maintains the health and wellness of our workforce. A healthy workforce is more productive, consumes more goods and services, and engenders a broader tax base. Most healthcare services will continue to be provided by private insurance carriers (Tier 2). 

While measured in its approach, LLAPA will promote a stronger economy and significantly lower the cost of healthcare with a three-pronged plan of attack:

(1) Modernize the System
  • By fast-tracking interoperability between healthcare entities, outcomes improve and costs are reduced by 10-20% ($300b - $600b) nationwide. 
  • Healthcare administration accounts for 25% of total costs. Automating only half of administrative tasks would save $400 billion dollars per year.
  • The updated infrastructure will improve data analysis capabilities and facilitate new provider payment models, such as value-based care and DRG models, that lower costs and improve outcomes.
(2) Maintain a Healthy Population - Preventable chronic disease, addiction, and mental illness cost the US more than a trillion dollars per year in direct and indirect healthcare costs. Implementing prevention, education, and awareness programs will save billions of dollars each year.

(3) Pass Legislation to Stop the Price Gouging of Drug Companies - Outlawing the drug companies' anti-competitive practices such as product hopping and patent thicketing will allow more competition and lower prices.

The bedrock of capitalism is a healthy and well-trained workforce. By modernizing our healthcare system and changing the paradigm from treating the sick to caring for the well, we enable a healthier population and a more productive workforce. This increase in productivity will foster a robust economy which will in turn ensure the sustainability of LLAPA. This commonsense approach to reforming our healthcare system addresses the root problems and results in increased access, improved outcomes, and lower costs.
